Nurse Care Coordinator
Essential Responsibilities Include: Creates an evidence-based care plan for providers and an action plan for patients and caregivers. Identifies the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs and individualizes the patient’s plan of care based on the needs identified. Promotes patient self-management and disease and behavior modification interventions. Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits. Manages high risk patient care, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting. Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans. Coordinates the efforts of all care providers (hospitals, specialists, rehab facilities, home care, hospice, and social service agencies). Smoothing transitions between sites of care, (e.g., transitions into and out of hospitals). Educates and supports family caregivers. Facilitates access to community resources. Document problems, nursing assessments, appropriate goals, care provided, care interventions and patient and family outcomes achieved from intervention and care provided.
Qualifications: Current Commonwealth of Virginia Registered Nurse or Licensed Practical Nurse license. Completion of a nursing program from an accredited institution required. Minimum of two years clinical experience at an acute care or ambulatory setting preferred. Community or public health experience highly preferred. BLS required. Must be able to work independently, have demonstrated patient care planning and patient teaching skills, and strong written and verbal skills. English/Spanish fluency preferred.
Clinical Access Nurse
Overall Responsibilities: He/she promotes health and wellness in the community through the provision of professional nursing care at HealthWorks for Northern Virginia through telephonic patient triage and patient education. He/she utilizes telecommunications and information technologies to include electronic medical record and telephone system to help eliminate distance barriers and provide health care services. He/she partners with front desk and other HW staff to ensure accurate patient scheduling and delivery of appropriate services as it affects the clinical staff and clinical services.

Patient Care Coordinator
Responsibilities Include: Schedules patient appointments and overdue preventive visits in coordination with provider and nurse. Schedules and tracks specialty and lab referrals as ordered by the provider. Answers and dispatches provider line calls as per Healthworks provider line protocol. Arranges transportation for patients as needed according to established process. Responsible for distributing, scanning and collecting incoming and outgoing mail and records for assigned providers. Scans all patient related paper documents into patient electronic medical records daily. Empties the fax inbox and attach documents to patient electronic medical records daily. Verifies that all patients’ medical records are received within 2-3 business days and obtains any records that are needed from previous medical offices, hospitals, or specialists. Requests/provides documents per physician request.
Qualifications: High School Diploma or GED required. Associates Degree preferred. Two (2) years of progressive work experience in customer relations and office administration, preferably in a health care setting, required. Exemplary customer service and cultural competence skills required.
Medical appointment scheduling, knowledge of medical terminology and experience with electronic medical record (EMR) required. Public health or community health experience is a plus.
Must enjoy working both autonomously and in collaboration with a team. Proficient computer and EMR skills required.
Due to patient population, bilingual English/Spanish skills are required.
Certified Application Counselor
This position is responsible for providing education to patients and others about the Health Insurance Exchange Marketplace qualified health programs, as well as other programs such as Medicaid and Medicare, and helping to facilitate the submission of applications as appropriate.
Qualifications: Associate Degree or equivalent education and/or work experience required; Excellent computer skills required with knowledge of programs such as Excel; Excellent customer service and communications skills, ability to work independently and a positive history of work attendance required; Access to reliable transportation required; Must enjoy working with people and problem solving to ensure high degree of satisfaction with the delivery of services; Written and verbal English/Spanish fluency required; Recent work experience in a health care setting, knowledge of human services, community resources and community outreach preferred.
Clinical Dental Assistant
Location: Leesburg and Reston
Growing dental team looking to add a Clinical Dental Assistant two (2) days per week.
Qualifications: At least one (1) year of proven chair-side dental experience and x-ray certification required. Upbeat, professional and dependable with a strong desire to learn. Bilingual English/Spanish skills are a plus.
Dental Assistant 1
Overall Responsibilities: This is a highly visible position having a great deal of customer/staff contact and interaction. Position responsible for assisting the dentist(s) and dental hygienist(s) in the direct provision of primary dental care services to patients of the Center. The Dental Assistant 1 is responsible for sterilization, preparation and inventory control of dental instruments and supplies. Assists in assuring chairside documentation is complete i.e. consent forms, perio and exam charts. Utilizes knowledge of medical/dental terminology to assist in answering patient’s questions and to communicate with patients as needed Provides interpretation services as needed. Experience with Electronic Health Record preferred. This is a customer service position and critical in maintaining a positive, supportive relationship with the team, patients and the larger community.
Qualifications/Requirements: High School Diploma or GED required. Completion of accredited course in Dental Assisting required. Current CPR(BLS) required. Dental Radiation Safety certificate preferred. Must have a positive history of work attendance and punctuality. Computer skills required. Ability to multi-task and to prioritize assignments and responsibilities to ensure compliance with established deadlines and protocols. Bilingual English/Spanish verbal and written communication skills preferred.
