Step 2: Sliding Fee Discount Program

For patients who do not qualify for health care coverage, we offer health care through our Sliding Fee Discount Program based on household income and family size.

Patients whose income is under 200% of the Federal Poverty Level will qualify for the Sliding Fee Discount Program.  No patient will be denied care due to an inability to pay.

Step 3: What to Bring With You

If you DO NOT have medical insurance and want to be considered for discounted fees, an enrollment appointment will be made for you. You are required to bring all of the information below along with the registration form to your enrollment appointment.

The average time it takes to complete an application with our Enrollment Coordinator is approximately 30 minutes.
