Please visit the front desk of either one of our two centers to pick up a new patient packet and to schedule your enrollment appointment. Or, print out the forms and complete the steps below.

Step 2: Sliding Fee Discount Program

Patients whose income is under 200% of the Federal Poverty Level will qualify for the Sliding Fee Discount Program.  No patient will be denied care due to an inability to pay.

Step 3: What to Bring With You

We can speak with you about our discounted fees if you are insured or uninsured, and your income level is below 200% of the Federal Poverty level.  To do so please bring the information below to your enrollment appointment.
